The following is a list of selected forms for various enrollment and academic petitions. It’s important to note that a submission of any petition is a request for an exception, not an approval. Each petition goes through a rigorous review process. Therefore, your petitions need to be exceptional in nature and come with a rare and compelling justification.

If you need to add or drop a course or change your grading basis after the Friday, second week deadline (including after the end of the quarter in which you took the course), you should file this petition.

To consider a Late or Retroactive Enrollment Petition, the Herb Alpert School of Music requires:

  • Approval of the course instructor
  • A personal justification explaining why you were unable to add/drop the class or change the grading basis by the regular deadline and why your were unable to file the petition until now. The following are insufficient reasons for a petition to change grading basis: poor or superior academic performance, or poor judgement in choosing classes.
  • Any documentation that may support or verify your statement. NOTE: all retroactive changes require documentation.

If you are unable to obtain your instructor’s physical signature, we will accept a PDF or JPEG of an email from your instructor verifying the following:

  • Do you object to the student’s exception request for any reason?
  • Has the student attended class regularly?
  • Has the student completed any coursework or exams to date?

Please note that the answers to the above questions must be explicitly stated, or your petition cannot be processed.  We recommend you cut and paste the above and include it in your email request to the instructor.  If these questions are not explicitly answered, we will not be able to process your petition.

You must be prepared to upload a PDF or JPG of your instructor’s verification when submitting your request.

Please be aware that approval is NOT guaranteed, and is generally granted only for extenuating and documented circumstances. As a School of Music student, you are allowed three late enrollment petitions. Any petition after the third and any retroactive petitions are subject to far more scrutiny in review.

Please also note that study list petitions cannot be processed if you have holds that prevent enrollment transactions.    

After submission, you will not receive confirmation. Each petition is reviewed thoroughly by the School’s petitions committee. Petitions are not reviewed upon submission, but periodically by the committee. Once the petition is reviewed, you will receive an email regarding the status of your petition to see if it is approved, denied, or requires further information or action.

When to use: An add/drop/grading basis change is late if submitted after the Friday of second week deadline but before the last day of instruction in Week 10. An add/drop/grading basis change is retroactive if submitted after the last day of instruction.
Deadline: Last day of instruction in Week 10. Graduation, but you are encouraged to submit your petition as soon as possible, as the School will take into account the length of time between the quarter in which you took the class and the date you are petitioning.

Students who wish to take more than 20 units in any quarter must submit a progress petition for such request. The petition should include the proposed study list for the quarter, the amount of units proposed, and the reason why the student is requesting approval for a unit increase. Petitions to enroll in over 22 units require a degree planner to demonstrate need. Students can use either the included planner or can make a copy of the below Google Sheet.

Eligibility: To be eligible for excess units [exceed 20 units in a regular quarter (fall, winter or spring) or exceed 16 units in summer], you must meet ALL of the following conditions:

  • Be in good academic standing
  • Have earned a GPA of = 3.0 in the two quarters directly preceding the quarter of the request
  • Students entering UCLA as a freshman are eligible to petition for excess units after establishing three quarters of residency (e.g. Fall of 2nd year).
  • Students entering UCLA as a transfer student are eligible to petition for excess units after establishing one quarter of residency (e.g. Winter of 1st year).

Although unit increase petitions can be submitted prior to the start of the quarter of the request, they cannot be fully approved until grades from the previous quarter have been officially posted.

Use this form to submit a petition for various academic requirement exceptions (substitution requests for a GE or major requirement, etc.). All requests must include a copy of relevant course syllabi, and official sealed transcripts (if the course was completed outside UCLA).

Use this form to submit a petition for various academic progress exceptions (adding a major/minor, extended time to degree, reduced fee program, etc.). All requests must include an updated degree planner signed by the appropriate academic advisor(s) unless it’s to extend IGETC completion or extend an incomplete grade.

Use this form to receive approval and clearance for participation in a UC Education Abroad Program. Clearance requests must also include relevant syllabi for the proposed program of study and an updated degree planner.
